Matriderm® as a biological scaffold in penile resurfacing: A single-centre case series

Introduction:
Penile resurfacing is a complex surgical challenge often employed for scarring and contractures arising from various conditions, including buried penis syndrome, Fournier's gangrene and cancer-related complications. Traditional methods, such as split-thickness skin grafts (STSGs) or free flaps are associated with graft failure, contracture and prolonged recovery, often necessitating multi-stage procedures. Novel approaches are needed to optimise outcomes.
Methods:
We conducted a retrospective, descriptive case series evaluating the use of Matriderm®, a bovine-derived acellular dermal matrix, combined with STSG for single-stage penile resurfacing. Thirty-six patients underwent 38 operations at the Norfolk and Norwich University Hospital between 2017 and 2025. Data collected included demographics, surgical indications, complications, cosmetic, functional and psychological outcomes.
Results:
Successful graft uptake was achieved in 92.1% of procedures. Two cases experienced partial graft failure and one complete graft loss, which were all managed conservatively. Where recorded, good to excellent cosmetic outcomes were reported in 24 of 31 procedures (74.4%). Functional assessments (urinary and erectile functions) improved in 32 of 33 procedures (97.0%) with available data. Psychological outcomes were available for 12 procedures, with 9 (75%) demonstrating improvements in self-esteem and body image. One patient underwent three resurfacing procedures for recurrent scarring, with no graft loss or procedure-related complications across any operation.
Conclusion:
Matriderm® combined with STSG appears to be a feasible single-stage option for penile resurfacing, with favourable integration rates and encouraging cosmetic, functional and psychological outcomes. Further prospective studies are recommended to validate these findings across broader patient cohorts and to assess long-term outcomes.
