Inflammatory Bowel Disease V: Surgical Management01:21

Inflammatory Bowel Disease V: Surgical Management

800
Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Inflammatory Bowel Disease I: Ulcerative Colitis

Introduction
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...

Peptic Ulcer Disease III: Clinical Manifestations and Diagnostic Studies

742
Peptic ulcer disease (PUD) presents with diverse symptoms depending on the location and severity of the ulcer. Clinical manifestations of peptic ulcer include dull pain and a burning sensation in the mid-epigastric region.
Few clinical manifestations differentiate gastric ulcers from duodenal ulcers. Distinctions in the location, timing, and pain relief are crucial for healthcare providers in differentiating between gastric and duodenal ulcers during clinical assessments.

Inflammatory Bowel Disease III: Diagnostic Studies and Management I-Nutritional Therapy

998
Various diagnostic tests are employed in the diagnostic process for Inflammatory Bowel Disease (IBD), particularly to differentiate between Crohn's disease and ulcerative colitis.
Diagnostic studies
A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...

Endoscopic Procedures II: Colonoscopy

1.0K
The colon, or large intestine, is the final segment of the digestive system. Its primary functions include absorbing water and vitamins produced by gut bacteria and transforming waste from liquid to solid to form stool. In adults, the large intestine is approximately 5 feet long and consists of four main sections:

Assessment of the Rectum and Anus

1.6K
Evaluating the rectum and anus plays a crucial role in conducting a thorough physical examination of the gastrointestinal system. Although it may be uncomfortable and often embarrassing for the patient, it holds immense diagnostic value, particularly in detecting gastrointestinal diseases and abnormalities. This guide will explain how to perform this assessment using inspection and palpation methods.
Rectal Inspection
Begin by inspecting the perianal and anal areas for color, texture, rashes,...

Summary

A new Simple Endoscopic Score for Ulcerative Colitis (SES-UC) was developed using vascular pattern and ulcers. This simplified index shows comparable accuracy to existing tools for assessing ulcerative colitis disease activity.

Area of Science:

  • Gastroenterology
  • Endoscopic assessment
  • Inflammatory bowel disease

Background:

  • Endoscopy is the gold standard for evaluating ulcerative colitis (UC) disease activity.
  • Current endoscopic indices, like the Mayo Endoscopic Subscore (MES), have limitations.
  • Existing indices lack widespread adoption due to feasibility issues.

Purpose of the Study:

  • To develop a simple, reliable endoscopic index for UC.
  • To compare the MES, Ulcerative Colitis Endoscopic Index of Severity (UCEIS), and Ulcerative Colitis Colonoscopic Index of Severity (UCCIS).
  • To examine agreement between colonoscopy and sigmoidoscopy for UC assessment.

Main Methods:

  • Video recordings of colonoscopies from UC patients were segmented.
  • Gastroenterologists scored video segments using disease activity descriptors.
  • Collected data included clinical, fecal, and mucosal samples.

Main Results:

  • The Simple Endoscopic Score for Ulcerative Colitis (SES-UC) was developed from vascular pattern and ulcer descriptors.
  • SES-UC demonstrated reliability and accuracy comparable to MES, UCEIS, and UCCIS.
  • Sigmoidoscopy missed proximal UC disease in up to 38% of patients.

Conclusions:

  • The SES-UC is a simple, reliable endoscopic index for UC.
  • SES-UC performance is similar to, and in some aspects better than, existing indices.
  • SES-UC is suitable for clinical practice and research in UC.
