Summary

Care transitions from the emergency department (ED) are risky for patients with diagnostic uncertainty. We developed and validated the ED Transitions (EDT) measure to assess care quality and improve patient safety during these critical periods.

  • A mixed-methods study involving item generation by experts, cognitive interviewing (n=11), and large-scale validation (n=301).
  • Exploratory structural equation modeling (ESEM) was used to determine the factor structure.
  • Convergent and discriminant validity were assessed using the Care Transition Measure (CTM-3) and Communication Assessment Tool-Teams (CAT-T).

Main Results:

  • An iterative refinement process resulted in a 15-item measure, which was further validated on 301 patients.
  • Exploratory structural equation modeling identified a final 10-item, two-factor structure for the ED Transitions (EDT) measure.
  • The EDT measure demonstrated excellent reliability (0.93) and positive correlations with CTM-3 and CAT-T, indicating strong validity.

Conclusions:

  • The newly developed ED Transitions (EDT) measure possesses content validity, structural validity, convergent validity, discriminant validity, and high internal consistency.
  • The EDT measure is a validated patient-reported outcome measure suitable for research and clinical use.
  • This tool can facilitate a better understanding of the impact of ED interventions on the quality of care transitions for patients facing diagnostic uncertainty.
