Background:

  • Comprehensive genomic profiling (CGP) is established for metastatic castration-resistant prostate cancer.
  • The role of genomic profiling in localized prostate cancer (PC) is not well-defined.

Purpose of the Study:

  • To explore associations between somatic genomic alterations and biochemical recurrence (BCR) and recurrence-free survival (RFS) in localized PC.
  • To evaluate the utility of an in-house CGP platform for localized PC.

Main Methods:

  • Analyzed DNA from 314 localized PC surgical specimens using an in-house CGP platform (164 cancer-related genes).
  • Examined associations between genomic alterations and BCR/RFS post-radical prostatectomy.

Main Results:

  • Six genes (PTEN, BRCA2, POLD1, ERBB3, MYC, SETD2) were frequently altered in patients with BCR.
  • Alterations in these genes were linked to higher pathological T stage, increased BCR rates (27.1% vs. 6.4%), and worse RFS.
  • Genomic alterations were independently associated with worse RFS in multivariate analysis.

Conclusions:

  • Somatic genomic alterations identified at prostatectomy correlate with early recurrence in localized PC.
  • These alterations may serve as potential biomarkers for risk stratification and management.
  • Further validation in independent cohorts is necessary to confirm these findings.
