From Canvas to CT: How Artists Anticipated the Craniofacial Surgical Gaze

Abstract:
Craniofacial surgeons are trained to interpret the human face through its underlying skeletal framework, professional mode of vision symbolically reflected in the skull displayed on the cover of the Journal of Craniofacial Surgery. Although contemporary imaging technologies such as computed tomography (CT) facilitate this structural perspective, the historical origins of this gaze have not been fully explored. This study examines the cultural and artistic evolution of skull representation, tracing the transition from symbolic usage to anatomic and structural interpretation, and explores how this trajectory anticipated modern craniofacial surgical thinking. Key artworks from the seventeenth to nineteenth centuries were analyzed, including emblem books, anatomic paintings by Rembrandt, surgical scenes by Eakins, and skull studies by Cézanne. Seventeenth-century emblem books used skulls primarily as moral symbols rather than anatomic structures. During the Renaissance and early modern periods, artists gradually shifted toward empirical observation, as reflected in public dissections and surgical depictions. Cézanne's late skull paintings represent a critical conceptual bridge, treating bone as volumetric architecture rather than allegory. This approach closely parallels the contemporary surgical gaze, which prioritizes skeletal structure over surface appearance. The findings suggest that the anatomic perspective central to modern craniofacial surgery did not emerge abruptly with technological innovation but evolved through centuries of visual culture. Artists unknowingly trained human perception to recognize skeletal structure beneath soft tissue long before the advent of CT imaging. The skull depicted on the cover of JCFS thus represents not mortality, but professional identity-declaration of how craniofacial surgeons see the human face.
