Gastrointestinal Motility Disorders01:20

Gastrointestinal Motility Disorders

1.5K
Gastrointestinal or GI motility disorders are characterized by irregular gastrointestinal tract movements, disrupting food transit from the mouth to the anus. They are caused by damage or dysfunction in gut muscles or nerves. These disorders can cause symptoms such as severe constipation, diarrhea, abdominal pain, and swallowing difficulties. Disorders can affect any segment of the GI tract and range widely in severity, from common conditions like GERD to life-threatening conditions like...

Irritable Bowel Syndrome I: Introduction

1.4K
Irritable Bowel Syndrome (IBS) is characterized by functional disturbances in the gastrointestinal system, presenting a cluster of symptoms without evident structural or biochemical abnormalities. It primarily affects the large intestine and may cause abdominal pain, bloating, excessive gas, diarrhea, constipation, or both.
IBS is a chronic condition that can persist over a long period or recur frequently.
The pathogenesis of IBS involves a complex interplay of the following factors:

Assessing the gastrointestinal (GI) system is a complex process that begins with collecting subjective data. This data, collected through patient interviews, provides crucial insights into the patient's health history, perception patterns, and lifestyle habits, all contributing significantly to GI health.
Health Perception Patterns
Health perception patterns offer valuable insights into a patient's lifestyle habits and how they may impact their GI health. These patterns include:

The gastrointestinal tract is susceptible to various disorders. If the lower esophageal sphincter is damaged, stomach acid can flow back into the esophagus, causing irritation and inflammation of the lining. This condition is called gastroesophageal reflux disease (known as heartburn) and may cause chest pain and difficulty swallowing. In the stomach, prolonged use of nonsteroidal anti-inflammatory drugs like aspirin, chronic alcohol consumption, bacterial infections such as Helicobacter...

Area of Science:

  • Pediatric Gastroenterology
  • Child Psychology
  • Behavioral Science

Background:

  • Functional gastrointestinal disorders (FGIDs) are prevalent in childhood.
  • Psychological factors associated with pediatric FGIDs are not well understood.
  • Understanding these correlates is crucial for effective management.

Purpose of the Study:

  • To compare emotional and behavioral functioning in children with FGIDs.
  • To contrast FGID patients with those having somatic gastrointestinal diseases and healthy controls.
  • To identify psychological correlates impacting difficulties in FGID patients.

Main Methods:

  • Recruited children aged 2-12 years at a university children's hospital.
  • Categorized participants into Functional, Somatic, and Healthy control groups based on Rome IV criteria.
  • Administered the Strengths and Difficulties Questionnaire (SDQ) and Patient Health Questionnaire (PHQ-4) to parents.

Main Results:

  • Analyzed data from 218 children (Functional n=66, Somatic n=62, Healthy n=90).
  • Children with FGIDs exhibited significantly higher SDQ total difficulties compared to both control groups (p < 0.001).
  • Elevated difficulties were most prominent in emotional and hyperactivity domains, with 45.5% of FGID children exceeding cut-offs.

Conclusions:

  • Emotional and behavioral difficulties are substantially higher in pediatric FGIDs than in somatic gastrointestinal diseases.
  • This indicates a significant psychosocial burden associated with FGIDs in children.
  • Integrated pediatric gastroenterology care must incorporate psychological assessments and interventions.
