Balancing Regulations and Patient Needs: A Typology of Nursing Flexibility

Flexibility lies at the heart of person-centered care, enabling nurses to respond to the particularities of patients' needs, preferences, and circumstances. Yet hospital work unfolds within highly regulated systems designed to ensure safety and consistency, often constraining individualized practice. This tension sometimes positions nurses between organizational regulations and the imperatives of care, a space documented in the literature but still posing important questions about how nurses manage competing demands. This study explores how nurses manage conflicts between institutional rules and patient needs and develops a typology of the strategies they use to do so. Data were generated through semi-structured interviews and shadowing observations in internal medicine, surgical, neurological, and orthopedic wards across four hospitals. Findings show that nurses' strategies align along two axes: procedure orientation-patient orientation and personalization-depersonalization. Nurses moved fluidly among these modes, revealing flexibility as a dynamic practice rather than a fixed trait. The typology offers a conceptual lens for understanding how nurses enact discretion within highly standardized systems. Recognizing these patterned responses can help organizations support professional judgment, creating space for safe, responsive care without undermining institutional integrity.
