Lower extremity amputations in patients with diabetes mellitus and peripheral arterial disease carry significant risks of reamputation. This article highlights strategies to optimize outcomes, including meticulous surgical technique, aggressive infection and ischemia management, and systemic optimization through glycemic control and smoking cessation. Pressure redistribution and offloading are also critical to prevent reamputation. Lastly, close monitoring and follow-up in the postoperative period facilitates early detection of skin changes. This holistic approach is essential to improve patient outcomes and quality of life.
