Background:

Abdominal pregnancy involves extrauterine implantation outside the uterus, fallopian tubes, and ovaries.
Splenic pregnancy is an exceedingly rare subtype of abdominal pregnancy.

To report a case of splenic pregnancy diagnosed via advanced imaging.
To highlight the diagnostic difficulties and imaging modalities for splenic pregnancy.

A 24-year-old female presented with symptoms suggestive of ectopic pregnancy.
Transvaginal ultrasound failed to locate an intrauterine pregnancy; abdominal ultrasound revealed a splenic anomaly.
