Extracranial metastasis in histologically benign meningiomas: unpredictable behavior in WHO grade 1 tumors.

Background:
WHO grade 1 meningiomas typically exhibit indolent behavior, and extracranial metastasis is exceedingly rare.
Observations:
The authors present 2 unusual cases of recurrent grade 1 meningiomas with pulmonary metastases, one with additional spinal involvement. Both patients underwent multiple resections and radiotherapy, with histopathology consistently confirming grade 1 disease.
Lessons:
These cases highlight the paradox of histologically benign meningiomas exhibiting systemic dissemination, emphasizing the importance of clinician awareness and diagnostic suspicion in the presence of new systemic lesions, particularly in recurrent or atypical presentations. https://thejns.org/doi/10.3171/CASE265.
