Background:

  • Malignant spinal cord compression (MSCC) is a severe complication of cancer.
  • Radiotherapy is a primary treatment for MSCC, but outcomes vary.
  • Identifying prognostic factors is crucial for optimizing patient selection and treatment planning.

Purpose of the Study:

  • To identify prognostic factors for functional outcome in patients with MSCC treated with radiotherapy.
  • To evaluate treatment effectiveness based on performance status changes.

Main Methods:

  • Retrospective analysis of 295 patients with MSCC treated with radiotherapy (January 2017 - December 2022).
  • Data collected included patient, tumor, and treatment characteristics, and functional status before and after treatment.
  • Treatment effectiveness defined by maintained or improved performance status.

Main Results:

  • Overall treatment effectiveness was 44.7%.
  • Patients surviving >1 month post-radiotherapy had significantly better functional improvement (60.5% vs. 16.5%).
  • Multivariate analysis identified pre-treatment performance status (1-2), myeloma/lymphoma, MRI at diagnosis, and absence of motor deficits as predictors of better functional outcome.

Conclusions:

  • Prognosis for MSCC patients remains poor.
  • Improved patient stratification is needed to better assess radiotherapy benefits.
  • Further research should focus on refining patient selection for radiotherapy in MSCC.
