Background:

  • Short bowel syndrome (SBS) necessitates strategies for intestinal regeneration.
  • Spring-mediated distraction enterogenesis (SMDE) uses mechanical stretch to create new intestinal tissue.
  • Multiple SMDE procedures may be needed to achieve sufficient bowel length for SBS patients.

Purpose of the Study:

  • To assess the feasibility of repeated SMDE within the same intestinal segment.
  • To evaluate the efficacy of sequential spring insertions for intestinal lengthening.

Main Methods:

  • A porcine model (Yucatan pigs) was used to test repeated jejunal lengthening.
  • Initial nitinol springs were inserted, followed by removal and insertion of stronger springs in previously lengthened segments.
  • Histological and morphometric analyses were performed on collected tissue samples.

Main Results:

  • SMDE significantly increased jejunal length compared to controls.
  • Both initial and subsequent stronger springs achieved equivalent lengthening.
  • Repeated lengthening enhanced crypt depth, mucosal thickness, muscularis thickness, and serosal perimeter.

Conclusions:

  • Repeated SMDE is feasible in a porcine model, successfully relengthening previously treated jejunal segments.
  • This approach suggests potential for multiple distraction procedures to maximize bowel length in SBS patients.
  • Repeat SMDE may offer a greater increase in bowel length than single procedures for managing short bowel syndrome.
