Objective:
This study evaluated the accuracy of immediate implant placements performed by operators with different experience levels in anterior and posterior maxillary models using dynamic computer-assisted implant surgery.
Materials And Methods:
Twelve participants (six novice and six experienced operators) placed 120 implants in 3D-printed maxillary resin models designed to simulate immediate implant placement scenarios. Each operator placed five aggressive, fully self-tapping thread implants and five apical-aggressive thread implants targeting either anterior or posterior regions. Accuracy was assessed by measuring angular, 3D platform, 3D apex, and depth deviations between the planned and postoperative positions using cone-beam computed tomography imaging and specialized implant planning and evaluation software. Statistical analyses were conducted using linear mixed-effects models, with significance set at p < 0.05.
Results:
In the posterior region, novice operators showed significantly greater deviations in all parameters compared with experienced operators. In the anterior region, novices exhibited significantly higher angular deviation alone. Both groups demonstrated greater angular deviation when placing implants in the anterior region than in the posterior region. Regarding implant design, apical-aggressive thread implants were associated with greater angular deviation than fully self-tapping thread implants, especially when experienced operators worked in the anterior area and novice operators worked in the posterior region.
Conclusions:
Operator expertise plays a critical role in achieving accurate immediate implant placement, especially in the posterior maxilla. Implants with an aggressive, fully self-tapping design offer improved angular accuracy compared to apical self-tapping implants, suggesting that both implant design and operator experience could influence surgical outcomes.
