Flap-based nasal reconstruction for skin cancer offers good functional and aesthetic outcomes, with no recurrences in this patient group. Patient-reported outcomes highlight oncologic anxiety, necessitating further research.

Area of Science:

  • Plastic Surgery
  • Oncology
  • Patient-Reported Outcomes

Background:

  • Nasal reconstruction after skin cancer excision presents challenges in balancing form, function, and oncologic safety.
  • Patient-reported outcomes (PROs) are crucial for evaluating postoperative success beyond surgical metrics.

Purpose of the Study:

  • To retrospectively evaluate oncologic, surgical, and patient-reported outcomes in patients undergoing nasal reconstruction post-skin cancer excision.
  • To assess the effectiveness of different reconstructive techniques, particularly flap-based approaches.

Main Methods:

  • Retrospective cohort study of 60 patients (2019-2022) at University Hospital "G. Martino", Messina, Italy.
  • Reconstructive techniques included direct closure, skin grafts, and local/regional flaps.

  • PROs assessed using a questionnaire adapted from FACE-Q, NOSE, and SCaFF domains; oncologic outcomes and complications recorded.
  • Main Results:

    • Basal cell carcinoma was most common (55%); nasal ala, dorsum, and tip were most affected.
    • No recurrences observed in flap-reconstructed subgroup; recurrences seen in non-flap groups.
    • High patient satisfaction with function (93% no obstruction) and aesthetics (63% satisfactory/very satisfactory); patient-perceived recurrence risk (38%) higher than confirmed (15%).

    Conclusions:

    • Flap-based nasal reconstruction in an elderly cohort demonstrated favorable patient-centered outcomes and low complication rates.
    • A significant gap exists between patient-perceived and confirmed recurrence, indicating oncologic anxiety.
    • Further prospective studies with validated PROMs are needed for standardized outcome comparison and clinical guidance.
