T2-weighted imaging with flex water reconstruction significantly improves visualization of parathyroid adenomas compared to T1-weighted imaging. This finding supports a targeted MR protocol for primary hyperparathyroidism preoperative assessment.

- A limited MR protocol utilizing T2w FSE flex water is recommended for preoperative assessment in patients with pHPT undergoing PET/MR.
