Purpose of the Study:

  • To assess the association between socioeconomic factors (income, social vulnerability) and germline genetic testing referrals and completion in patients with advanced prostate cancer.
  • To identify predictors of genetic testing uptake in this population.

Main Methods:

  • Retrospective study of 1385 patients with stage III-IV PCa diagnosed between 2017-2022.
  • Analysis of median household income (gMHI) and social vulnerability index (SVI) based on census tract data.
  • Logistic regression models were used to evaluate associations with genetic referral and testing completion, adjusting for clinical factors.

Main Results:

  • Black patients and those with stage IV PCa were more likely to receive genetic referrals.
  • Patients in the middle gMHI tertile had lower odds of referral compared to the lowest gMHI tertile.
  • No significant association was found between gMHI, SVI, and genetic counseling referrals or testing completion; age and advanced stage predicted higher testing rates.

Conclusions:

  • Socioeconomic factors did not demonstrate a significant association with germline genetic testing uptake in this advanced PCa cohort.
  • The study suggests that socioeconomic barriers to genetic testing may be surmountable.
  • Clinical factors, specifically age and disease stage, are independent predictors for genetic testing completion in advanced prostate cancer.
