Evaluation of Serum Creatinine and Urea in Patients with Acute Stroke

Abstract:
Stroke is a leading contributor to mortality and long-term disability both globally and within Bangladesh. Post-stroke activation of the renal sympathetic nervous system may result in significant alterations in glomerular function. To explore this association, a cross-sectional comparative study was conducted from July 2023 to June 2024 in the Department of Biochemistry, Mymensingh Medical College, in collaboration with the Department of Medicine, Mymensingh Medical College Hospital, Bangladesh. The study was carried out to evaluate the status of serum creatinine and urea in patients with acute stroke for monitoring the disease progression. The study included a total of 120 participants. Out of them, 60 were selected as a case (both type of acute stroke patients) and another 60 apparently healthy individuals were selected as control. The subjects were selected based on inclusion and exclusion criteria by purposive and convenient sampling method Serum creatinine was measured using a colorimetric method based on the Jaffe's alkaline picrate reaction, while serum urea was estimated using the enzymatic colorimetric method following the Urease-Berthelot procedure from each sample. Statistical analysis was done using SPSS (statistical product and service solutions) version 26.0 windows package. Statistical significance of difference between Group I and Group II were evaluated by using Student's unpaired 't'- test among the significance was defined as p<0.05. Among the study groups, serum creatinine were 1.13±0.21 and 0.93±0.26 mg/dl in case and control respectively, serum urea were 37.70±10.00 and 16.57±5.45 mg/dl in case and control respectively. The mean serum creatinine and urea were significantly increased in acute stroke patients when compared to that of healthy individual. According to the findings of this study, renal impairment may have occurred in acute stroke.
