Background:

  • Chronic lymphocytic leukemia (CLL) and its treatments are linked to increased skin cancer risk.
  • Detailed long-term data on this association, including cancer progression and mortality, are limited.

Purpose of the Study:

  • To evaluate the risk of skin cancer, its subtypes, metastasis, and cancer-specific death in CLL patients.
  • To compare these risks against a control group without CLL.

Main Methods:

  • A nationwide, population-based, matched cohort study using Danish registry data (1990-2020).
  • 8,352 CLL patients were matched 1:5 with 41,760 controls.
  • Exclusion of patients with prior skin cancer or immunosuppression.
  • Analysis of absolute risks and hazard ratios using Cox regression models.

Main Results:

  • Patients with CLL had a 10-year skin cancer risk of 13.5% vs. 6.9% in controls (Absolute Risk Difference [ARD]: 6.6 pp).
  • Increased risks observed for basal cell carcinoma (ARD: 3.2 pp) and squamous cell carcinoma (ARD: 3.3 pp).
  • Higher risks of skin cancer metastasis (ARD: 0.6 pp) and cancer-specific death (ARD: 0.2 pp) in CLL patients.
  • All-cause mortality was significantly higher in CLL patients (56.3% vs. 39.3%).

Conclusions:

  • CLL patients exhibit a significantly increased risk of developing common skin cancers.
  • While skin cancer metastasis and death are more frequent in CLL, the absolute risk is low relative to high all-cause mortality.
  • The findings highlight the need for vigilant skin cancer surveillance in CLL patients.
