Aim:
To assess pediatric intensive care unit (PICU) nurses' knowledge, attitudes, and practices regarding post intensive care syndrome-pediatric (PICS-p) management, and to examine pathways linking these factors.
Methods:
A multicenter cross-sectional survey was conducted among 238 PICU nurses from 12 tertiary hospitals in China. A validated 35-item knowledge-attitude-practice (KAP) questionnaire was used. Descriptive statistics, multiple linear regression, and structural equation modeling were performed to identify factors associated with KAP and to examine the relationships among them.
Results:
Nurses demonstrated moderate levels of knowledge and practice, but relatively positive attitudes toward PICS-p management. Greater PICU experience, prior experience in caring for children with PICS-p, and comprehensive training were associated with higher KAP scores. Knowledge positively predicted attitudes (β = 0.43) and practices (β = 0.34). Attitudes also predicted practice (β = 0.44) and partially mediated the relationship between knowledge and practice. The model explained 18.6% of the variance in attitudes and 44.3% in practices.
Conclusions:
Although PICU nurses recognize the importance of PICS-p, gaps remain in their knowledge and implementation. Strengthening structured training and providing better institutional support may facilitate the translation of positive attitudes into consistent clinical practice, contributing to improved long-term outcomes for critically ill children.
Implications For Practice:
PICU nurses need clearer guidance and structured training to provide consistent PICS-p care. Integrating practical strategies such as safe early mobility, comfort measures, and regular communication with families can be added to daily routines. Supportive unit policies and interdisciplinary collaboration may help nurses apply these practices and improve children's recovery.
