An unusual presentation of cystic lymphangioma in the mesoappendix: a case report

Cystic lymphangioma of the mesoappendix is an extremely rare benign tumor of the abdominal cavity. Its clinical presentation is often nonspecific, and preoperative diagnosis can be challenging particularly in areas where hydatid disease is endemic. We report a case of cystic lymphangioma of mesoappendix discovered incidentally during surgery for a recurrent liver hydatid cyst. A pathological examination is the only way to provide definitive confirmation.

We present a case of a cystic lymphangioma in an unusual location: the mesoappendix. The patient was a 49-year-old woman who had undergone surgery 16 years earlier for a hydatid cyst of the liver. She consulted for chronic abdominal pain, with no impact on her general condition. The radiological assessment indicated the presence of an intra-peritoneal cyst, suggesting a recurrence of hydatid disease. The surgical team performed a complete removal of the cystic mass via median laparotomy. The histological examination of the cystic mass provided clear evidence to support the diagnosis of cystic lymphangioma of the mesoappendix.

Cystic lymphangioma of the mesoappendix is exceedingly rare and often discovered incidentally. Surgical excision is the treatment of choice and has an excellent prognosis. This case emphasizes the importance of thorough intraoperative exploration, especially in patients operated for recurrent abdominal pathologies such as liver hydatid cysts.
