    Summary

Transverse taeniamyotomy significantly reduced the mean motility index in patients with achalasia. This surgical intervention, even in early disease stages, shows promising results for improving esophageal motility.

    Area of Science:

    • Gastroenterology
    • Surgical Innovation
    • Esophageal Motility Disorders

    Background:

    • Achalasia is a primary esophageal motility disorder characterized by impaired relaxation of the lower esophageal sphincter and loss of esophageal peristalsis.
    • Current treatments aim to reduce the pressure gradient across the esophagogastric junction, but long-term efficacy and patient selection remain areas of investigation.

    Purpose of the Study:

    • To analyze the indications and outcomes of transverse taeniamyotomy in patients with achalasia.
    • To evaluate the impact of transverse taeniamyotomy on esophageal intraluminal pressure and motility.

    Main Methods:

    • A retrospective analysis of 11 patients who underwent transverse taeniamyotomy.
    • Intraluminal pressure recordings were performed before and one year after the surgical procedure.
    • Motility index was assessed under both natural and pharmacologic stimuli.

    Main Results:

    • Transverse taeniamyotomy demonstrated a significant decrease in the mean motility index one year post-operation.
    • The reduction in motility index was observed following both natural and pharmacologic stimuli, indicating improved esophageal function.
    • The study included 11 patients, providing a focused dataset for outcome analysis.

    Conclusions:

    • Transverse taeniamyotomy is an effective surgical option for managing achalasia.
    • The procedure leads to significant improvements in esophageal motility, even when performed in the early stages of the disease.
    • Consideration of transverse taeniamyotomy as an early treatment option for achalasia is warranted.
