Summary

Vaginal natural orifice transluminal endoscopic surgery (vNOTES) shows comparable perioperative complications to other minimally invasive approaches. This review supports vNOTES as a safe alternative for abdominal surgery (AS).

Background:

  • Minimally invasive abdominal surgery (AS) offers advantages over open procedures.
  • Various minimally invasive AS approaches exist, including vaginal, robotic, laparoscopic, and vNOTES.

Purpose of the Study:

  • To compare perioperative complications of vNOTES with other minimally invasive AS routes.
  • To review existing comparative literature on vNOTES versus other AS techniques.
  • To determine the prevalence of different minimally invasive AS routes in the US.

Main Methods:

  • Retrospective cohort study using the 2022 NSQIP database for female patients with benign diagnoses.
  • Included vNOTES, robotic, laparoscopic, and vaginal AS cases.
  • Conducted literature search using PubMed and ClinicalKey.

Main Results:

  • vNOTES (4.9%) was compared to vaginal (21.3%), robotic (37.1%), and laparoscopic (36.7%) AS.
  • No significant difference in composite perioperative complications between vNOTES and other routes (4.28-6.21%).

Conclusions:

  • vNOTES exhibits similar complication rates to established minimally invasive AS approaches.
  • Findings align with existing literature, supporting vNOTES as a viable surgical option.
