Background:

  • Slow-wave sleep (SWS), or stage N3, is often reduced in depression.
  • However, the variability of N3 architecture across different depressive phenotypes is not well understood.

Purpose of the Study:

  • To investigate if N3 sleep architecture (proportion, duration, latency) can identify clinically distinct profiles in individuals with major depressive episode (MDE).

Main Methods:

  • Ninety-seven adults with MDE underwent polysomnography to analyze N3 metrics (latency, duration, % of total sleep period (TSP), % of total sleep time (TST)).
  • Analyses included dimensional correlations with clinical measures and categorical comparisons (reduced, normal, increased N3).
  • Logistic regression identified distinguishing variables, with sensitivity analyses for medication effects.

Main Results:

  • Shorter N3 latency correlated with poorer sleep quality, anxiety, and depressive severity.
  • Increased N3 proportion (%TSP) linked to earlier hospitalization, younger age at first suicide attempt, and daytime sleepiness.
  • Reduced N3 proportion (%TSP) associated with more hospitalizations and depressive episodes.
  • Specific N3 duration and %TST variations correlated with symptom severity, hospitalization history, seasonality, chronotype, sex, and depression type (bipolar vs. unipolar).
  • A five-factor model accurately distinguished increased %N3 TST (AUC=0.863).

Conclusions:

  • N3 sleep architecture is not a uniform biomarker in depression; its variations define clinically meaningful profiles.
  • Sleep-based stratification holds potential for guiding precision care in MDE.
  • Psychotropic medication did not significantly affect the observed N3 patterns.
