Background:

Type 1 diabetes (T1D) technology use offers significant benefits but faces inequities among Hispanic and Black young adults.
These demographic groups are the fastest-growing populations with T1D in the U.S. and experience poorer health outcomes due to technology access barriers.

Purpose of the Study:

To explore the effectiveness of community health workers (CHWs) in addressing social needs and facilitating T1D technology navigation.
To improve the standard of care and health outcomes for high-risk young adults with T1D.

Main Methods:

The CATCH-T1D intervention integrates social needs management with T1D technology navigation, utilizing trained CHWs.
A hybrid effectiveness-implementation trial is ongoing, comparing CATCH-T1D to usual care in 130 Hispanic and/or Black young adults (18-35 years) with T1D.
CHWs provide information access, shared decision-making support, and health system-pharmacy coordination.

Main Results:

118 participants randomized (48% female, 67% Hispanic, 72% Medicaid, mean HbA1c: 9.2%).
CHWs conducted 476 follow-up encounters, resolving 24% of identified social needs.
High participant satisfaction reported, with increased engagement in diabetes self-management and improved trust in the healthcare system.

Conclusions:

The CATCH-T1D model effectively addresses social needs and T1D technology navigation simultaneously.
This specialized CHW approach shows potential as a scalable model for improving care and outcomes in underserved populations.
