Miliary tuberculosis in British Columbia.

I G Campbell

    Canadian Medical Association Journal

    Summary

Miliary tuberculosis diagnosis was often delayed until autopsy in elderly males, particularly those with other health issues. Early diagnosis and treatment are crucial for improving outcomes in tuberculosis patients.

    Area of Science:

    • Medicine
    • Infectious Diseases
    • Pulmonology

    Background:

    • Miliary tuberculosis is a severe form of tuberculosis characterized by widespread dissemination.
    • Diagnosis and treatment delays can significantly impact patient outcomes.

    Purpose of the Study:

    • To analyze the clinical characteristics and diagnostic patterns of miliary tuberculosis.
    • To identify factors associated with delayed diagnosis and mortality in miliary tuberculosis patients.

    Main Methods:

    • Retrospective review of 48 miliary tuberculosis cases registered between 1967 and 1971.
    • Comparison of patients diagnosed antemortem versus those diagnosed postmortem.

    Main Results:

    • 30 patients diagnosed during life received antitubercular therapy; 18 diagnosed at autopsy received none.
    • The autopsy-diagnosed group had more concomitant diseases and neglected diagnostic tests.
    • Elderly males, particularly Caucasians, were prevalent in the autopsy group, often presenting with insidious symptoms or fulminant illness.
    • North American Indians had a high incidence but lower mortality, likely due to younger age.

    Conclusions:

    • Delayed diagnosis of miliary tuberculosis is associated with specific demographic and clinical factors.
    • Diagnostic neglect and comorbidities contribute to poor outcomes.
    • Timely diagnosis and treatment are essential for managing miliary tuberculosis effectively.
