Reflex tearing during micturition and defecation is an unusual occurrence, with only a few documented cases in the literature. We present a case of a 46-year-old female who has experienced reflex tearing during both micturition and defecation since childhood. The patient sought advice from multiple ophthalmologists but found no effective solution. Unlike previously reported cases, where alternative management approaches were described, we initiated treatment with botulinum toxin injections into the lacrimal glands. The patient reported significant symptomatic improvement following treatment. Understanding this phenomenon and addressing the patient's concerns are essential for delivering optimal care for individuals with this condition.
