Telehealth - A Clinical Practice Initiative in Pediatrics
Insights
A telehealth program improved pediatric gastroenterology referrals by connecting primary care nurse practitioners with specialists. This initiative reduced wait times and enhanced patient care coordination.
Area of Science:
- Pediatric Gastroenterology
- Telehealth
- Healthcare Access
Background:
- Pediatric gastroenterology centers face high referral volumes from primary health care (PHC) for diagnostic and management clarification.
- Inefficiencies in the referral process can lead to delays in specialist care.
Purpose of the Study:
- To evaluate the effectiveness of a telehealth program connecting PHC pediatric nurse practitioners (PNPs) with tertiary pediatric gastroenterologists.
- To assess the impact of virtual consultations on patient management and access to specialized care.
Main Methods:
- A telehealth program facilitated weekly virtual meetings between PHC PNPs and a tertiary pediatric gastroenterology team.
- Data from 121 referred patients were discussed over a four-month period.
- PNPs were central to managing diagnostic methods, treatment, and follow-up.
Main Results:
- 55% of patients successfully scheduled timely appointments at the tertiary outpatient clinic.
- 20% of patients were managed at the PHC level based on virtual specialist recommendations.
- 25% of referral forms required clarification, highlighting communication needs.
Conclusions:
- The telehealth program effectively bridged primary and tertiary care, enhancing patient care coordination.
- Virtual consultations reduced waiting times for specialist assessments and improved care efficiency.
- The program facilitated online therapeutic orientation for a significant portion of referred patients.
Introduction:
Pediatric gastroenterology centers consistently receive high volume of referrals from primary health care (PHC) for diagnostic and management clarification.
Methods:
We present a telehealth program that connects PHC pediatric nurse practitioners with a tertiary team of pediatric gastroenterologists. During weekly virtual meetings over a four-month period, the health professionals discussed data from 121 referred patients.
Results:
The telehealth program enabled 55% of patients to schedule timely appointments at the tertiary outpatient clinic. Approximately 20% of patients were advised to remain at the PHC for management as recommended by specialists virtually. Approximately 25% of referral forms were sent back for clarification. Pediatric Nurse Practitioners (PNPs) played a central role in managing diagnostic methods, treatment decisions, and follow-up calls.
Discussion:
PNPs connected primary and tertiary care, thereby improving the effectiveness of patient care. The telehealth program reduced waiting times for specialist assessments, and it provided online therapeutic orientation for one-quarter of the patients discussed.

Specialized Care Centers and Settings-II
Rural health centers are specialized care facilities in remote locations with very few medical personnel. The primary care providers who run the centers are mostly Registered Nurse Practitioners. Here, emergency treatment is provided to critically ill or injured patients before they are transferred to the closest hospital. Fortunately, due to advancement in technology, many rural healthcare facilities and professionals have easy access to diagnostic and treatment...
