Ponseti-Based Hybrid Approach for Bilateral Teratologic Clubfoot With Congenital Peroneal Nerve Dysfunction: A Case

Case:
We report the case of a newborn boy with bilateral clubfoot associated with congenital peroneal nerve dysfunction who responded poorly to the standard Ponseti method. Successful initial correction was achieved through multiple tenotomies of the Achilles, posterior tibialis, flexor digitorum longus, and flexor hallucis longus tendons. Recurrence of equinus deformity without hindfoot varus was treated with posterior soft tissue release, during which all previously transected tendons were found to have completely regenerated.
Conclusion:
This case highlights the value of a Ponseti-based treatment combined with selective open soft-tissue release and documents spontaneous regeneration of flexor tendons following complete tenotomy in infancy.
