We report a case of mucinous cystic neoplasm that presented with intratumoral bleeding. The tumor showed unusual imaging morphology mimicking echinococcosis and other cystic liver lesions. A woman in her 60s with frequent contact with multiple dogs presented with an incidentally detected liver mass. Computed tomography and magnetic resonance imaging revealed a 53-mm lobulated hepatic mass with a central hematoma and peripheral cysts. Differential diagnoses included hemorrhagic cyst, echinococcosis, hepatocellular adenoma, mucinous cystic neoplasm, and metastatic tumors. Serologic testing for echinococcus antibodies was performed based on the imaging findings and yielded negative results. The patient underwent surgical resection, and the lesion was diagnosed as mucinous cystic neoplasm. No signs of recurrence were observed during the 5-year follow-up period.
