Background:

  • Systemic sclerosis is a multisystem autoimmune disease causing vasculopathy and fibrosis.
  • Ocular manifestations of systemic sclerosis are diverse and can affect vision.
  • Optical coherence tomography (OCT) is a key non-invasive imaging tool for retinal assessment.

Purpose of the Study:

  • To identify and characterize novel retinal imaging features in systemic sclerosis patients using OCT.
  • To compare retinal findings in systemic sclerosis patients with age-matched healthy controls.

Main Methods:

  • 41 systemic sclerosis patients and 38 healthy controls underwent comprehensive ophthalmologic exams.
  • Optical coherence tomography (OCT) was used to visualize retinal structures.
  • Key metrics included best-corrected visual acuity, central retinal thickness, and subfoveal choroidal thickness.

Main Results:

  • Systemic sclerosis patients showed a high prevalence of previously unreported OCT findings: drusen-like deposits (46.3%), wide-based foveal pits (63%), and epiretinal membranes (21%).
  • These abnormalities were significantly more common in systemic sclerosis patients compared to controls (p < 0.01).
  • Central retinal thickness was slightly reduced in patients, but not statistically significant.

Conclusions:

  • Systemic sclerosis is associated with a high frequency of underrecognized retinal findings.
  • Foveal contour abnormalities and drusen-like deposits are potential imaging biomarkers.
  • These OCT findings likely reflect the systemic microvascular and fibrotic disease burden.
