Summary

A child with diffuse intrinsic pontine glioma (DIPG) achieved a radiological complete response and long-term survival using radiotherapy and temozolomide. This case highlights a rare but possible positive outcome in DIPG treatment.

Background:

  • Diffuse intrinsic pontine glioma (DIPG) has a historically poor prognosis with limited radiological response to treatment.
  • Complete radiological response in DIPG is exceptionally rare, despite potential for transient clinical improvement.

Purpose of the Study:

  • To report a case of DIPG with a complete radiological response and prolonged survival.
  • To discuss the implications of this rare outcome for DIPG prognosis and treatment.

Main Methods:

  • A 9-year-old boy diagnosed with DIPG received concurrent radiotherapy and temozolomide, followed by adjuvant temozolomide chemotherapy.
  • Serial MRI scans were used to monitor tumor response.

Main Results:

  • Initial MRI showed a large pontine mass (35 mm diameter).
  • Post-radiotherapy MRI revealed significant tumor size reduction.
  • After 12 cycles of chemotherapy, MRI demonstrated complete regression of the pontine mass.
  • The patient achieved a complete radiological response and remained disease-free for 3.5 years, eventually succumbing to progressive disease 6 years after diagnosis.

Conclusions:

  • This case demonstrates that a complete radiological response and long-term survival are achievable in DIPG with radiotherapy and temozolomide.
  • Further research is needed to establish reliable models for assessing radiological response and predicting prognosis in DIPG.
