    Background:

    • Postpartum hemorrhage (PPH) is a major cause of maternal mortality.
    • Early identification of PPH risk during pregnancy is critical for effective management.
    • A quality improvement initiative implemented a prenatal PPH risk assessment tool within the electronic health record (EHR).

    Purpose of the Study:

    • To evaluate the implementation and effectiveness of an integrated prenatal PPH risk assessment tool in the EHR.
    • To assess adherence to screening protocols and identify patient risk factors.
    • To examine the impact of the risk assessment on clinical decision-making and patient outcomes.

    Main Methods:

    • A systemwide quality improvement initiative embedded a PPH risk assessment tool into the EHR in 2023.
    • Retrospective chart review assessed screening adherence across 28 outpatient obstetric clinics (October 2023 - June 2024).
    • Subset analysis examined risk levels, factors, recommendations adherence, and delivery outcomes for patients screened October-December 2023.

    Main Results:

    • 93.6% of 4,688 birthing individuals were screened for PPH risk.
    • 12.0% were moderate risk and 13.0% were high risk.
    • High-risk factors included prior PPH (55.4%) and cesarean delivery (16.5%); 92.8% received blood bank consultation, and 9.4% experienced PPH.

    Conclusions:

    • Integrating prenatal hemorrhage risk assessment into EHRs with standardized workflows facilitates timely interventions.
    • Automated alerts and multidisciplinary coordination improve clinical decision-making and care continuity.
    • This EHR-integrated approach has significant potential to improve maternal outcomes, especially in resource-limited settings.
