Abstract:
This monocentric study evaluated the real-world efficacy and safety of sonidegib in 17 patients with locally advanced (laBCC) or multiple (mBCC) basal cell carcinomas, characterized by complex surgical histories and comorbidities. The objective response was that 53% of patients achieved complete response, 29% partial response and 18% stable disease, with no documented progression during treatment (mean 7.3 months). Sonidegib showed significant efficacy in mBCC, with a mean reduction of 3.89 lesions per patient. Key findings indicate that prior surgery correlates with poorer response, while sonidegib effectively 'clears the field' of subclinical lesions. The safety profile was favourable, with predominantly mild and manageable adverse events. Despite the limited sample size, our findings confirm the significant efficacy of sonidegib in a frail pretreated population with multiple comorbidities. This study supports the use of sonidegib as a valuable therapeutic strategy for complex patients with high tumour burden or those ineligible for further surgery.
Insights
Sonidegib demonstrated significant efficacy and a favorable safety profile in treating advanced basal cell carcinomas (BCCs). This therapy effectively reduced tumor burden in complex, pre-treated patients, supporting its use for high-risk BCC cases.
Area of Science:
- Dermatology
- Oncology
Background:
- Basal cell carcinoma (BCC) is the most common human cancer.
- Locally advanced or multiple BCCs (laBCC/mBCC) present significant treatment challenges, especially in patients with complex surgical histories and comorbidities.
Purpose of the Study:
- To evaluate the real-world efficacy and safety of Sonidegib in patients with laBCC/mBCC.
- To assess Sonidegib's impact on tumor burden and identify factors influencing treatment response.
Main Methods:
- A monocentric study involving 17 patients with laBCC/mBCC.
- Real-world data collection on Sonidegib treatment, including objective response rates and adverse events.
Main Results:
- An objective response rate of 52.9% (Complete Response) and 29.4% (Partial Response) was observed.
- Sonidegib achieved a mean reduction of 3.89 BCC lesions per patient.
- Prior surgery correlated with a poorer response, but Sonidegib effectively addressed subclinical lesions.
Conclusions:
- Sonidegib shows significant efficacy and a favorable safety profile in a frail, pre-treated laBCC/mBCC population.
- It is a valuable therapeutic option for complex patients with high tumor burden or those ineligible for surgery.
