Background:
Deucravacitinib, an oral tyrosine kinase (TYK)2 inhibitor, has demonstrated efficacy and safety in phase III clinical trials for moderate-to-severe plaque psoriasis. However, real-world evidence remains limited.
Objectives:
To evaluate the effectiveness and safety of deucravacitinib in adult patients with moderate-to-severe plaque psoriasis treated in routine clinical practice.
Methods:
We conducted a retrospective, single-centre, real-world observational study. Consecutive adult patients with -moderate-to-severe plaque psoriasis who initiated deucravacitinib between December 2024 and December 2025 were retrospectively identified. At each follow-up Psoriasis Area and Severity Index (PASI), Dermatology Life Quality Index (DLQI), body -surface area (BSA), numeric rating scale (NRS) pruritus. In addition, ≥ 75% improvement in PASI (PASI 75), ≥ 90% improvement in PASI (PASI 90) and 100% improvement in PASI (PASI 100) response rates were also evaluated. Safety was assessed through adverse event reporting and routine laboratory monitoring.
Results:
In total, 46 patients completed the 24-week follow-up and were included in the study. Mean PASI decreased from 12.5 (SD 3.1) at baseline to 3.2 (SD 2.1) at week 24 (P < 0.001) and mean BSA decreased from 18.0% (SD 6.5%) to 5.0% (SD 3.6) (P < 0.001). NRS pruritus and DLQI also improved significantly over time (both P < 0.001). Of the 46 patients, PASI 75 was achieved by 70% (n = 32) at week 24, and PASI 90 and PASI 100 were achieved by 54% (n = 25) and 33% (n = 15), respectively. No serious adverse events or clinically relevant laboratory abnormalities were observed. Acne was the most frequent adverse event, occurring in 4 of 46 patients (9%), and was mild in all patients.
Conclusions:
In this real-world study, deucravacitinib showed significant and sustained effectiveness, together with a favourable safety profile, in patients with moderate-to-severe plaque psoriasis. These findings support its role as an effective oral targeted therapy in routine clinical practice.
