Major trauma is one of the greatest challenges in intensive care, often complicated by multiorgan failure. This case report describes the management of a 47-year-old patient with pelvic crush trauma, complicated by rhabdomyolysis, acute kidney injury, septic shock, and liver dysfunction. The therapeutic approach included the sequential use of extracorporeal blood purification techniques - high cut-off filters (EMIC2®), hemoadsorption (CytoSorb®), and multifunctional membranes (Oxiris®) - to provide renal support and to remove myoglobin, cytokines, endotoxins, and bilirubin. Timely and personalized integration of these technologies contributed to the full recovery of both renal and hepatic function, highlighting the importance of an intensive, multidisciplinary, and technologically advanced approach in the management of critically ill patients.
