The anti-EpCAM antibody EpAb3-5 improves circulating tumor cell (CTC) detection in head and neck squamous cell carcinoma (HNSCC). Low EpCAM expression detected by EpAb3-5 may aid risk stratification in early-stage HNSCC patients.
Area of Science:
- Oncology
- Immunology
- Biomarkers
Background:
- Epithelial cell adhesion molecule (EpCAM) is a transmembrane protein implicated in cell adhesion and cancer progression.
- Anti-EpCAM antibodies, such as EpAb3-5, are utilized for detecting circulating tumor cells (CTCs).
- The clinical and prognostic significance of EpAb3-5 in head and neck squamous cell carcinoma (HNSCC) requires further elucidation.
Purpose of the Study:
- To evaluate the sensitivity of EpAb3-5 for CTC detection in HNSCC compared to a commercial platform.
- To assess the correlation between EpAb3-5-detected EpCAM expression and clinicopathologic features in HNSCC.
- To determine the prognostic value of EpAb3-5-detected EpCAM expression, alone and in combination with tumor staging, for patient survival.
Main Methods:
- Retrospective analysis of 66 HNSCC patients.
- Enumeration of CTCs using EpAb3-5 and a MACS EpCAM-based platform.
- Immunohistochemical assessment of tumor EpCAM expression using EpAb3-5.
- Kaplan-Meier and Cox regression analyses for survival outcomes.
Main Results:
- EpAb3-5 demonstrated significantly higher CTC detection sensitivity than the MACS platform (p < 0.001).
- Tumor EpCAM expression correlated with nodal status (p < 0.01) but not T classification or overall stage.
- Low EpCAM expression, assessed by EpAb3-5, predicted better overall survival in early-stage HNSCC patients (p < 0.05).
Conclusions:
- EpAb3-5 offers enhanced CTC detection sensitivity and provides prognostic information in HNSCC when integrated with tumor staging.
- EpCAM immunoreactivity assessed by EpAb3-5 may refine risk stratification in early-stage HNSCC.
- Further validation in larger prospective cohorts is warranted to confirm these exploratory findings.
