Treatment Resistant Cancers02:56

Treatment Resistant Cancers

Cancer is the second leading cause of death in the United States. A cancer cell is genetically unstable and hence can mutate faster. They can also modify their microenvironment and escape immune surveillance. The difficulties in treating cancer are further compounded by the emergence of rapid resistance to anticancer drugs. The most common ways to attain resistance in cancer cells include alteration in drug transport and metabolism, modification of drug target, elevated DNA damage response, or...

Area of Science:

  • Oncology
  • Immunotherapy
  • Clinical Research

Background:

  • Advanced non-small cell lung cancer (NSCLC) presents significant treatment challenges.
  • Pembrolizumab monotherapy is a key treatment option for NSCLC.
  • Real-world data is crucial for understanding treatment effectiveness in diverse settings.

Purpose of the Study:

  • To evaluate the real-world effectiveness and safety of first-line pembrolizumab monotherapy.
  • To assess outcomes in Vietnamese patients with advanced NSCLC.
  • To identify predictors of treatment response and survival.

Main Methods:

  • Multicenter retrospective cohort study.
  • Inclusion of patients with locally advanced or metastatic NSCLC receiving first-line pembrolizumab.
  • Primary endpoints: progression-free survival (PFS), overall survival (OS), and safety profile.

Main Results:

  • 73 patients included; median age 69; 75.3% ECOG PS 0-1; 86.3% PD-L1 TPS ≥ 50%.
  • Overall response rate 60.3%; disease control rate 79.5%. Median PFS 11.3 months; median OS 25.4 months.
  • Predictors of inferior OS: never-smoking status, squamous histology, low PD-L1 expression. Grade 3 toxicity in 4.2%.

Conclusions:

  • First-line pembrolizumab monotherapy is effective and safe for advanced NSCLC in Vietnam.
  • High PD-L1 expression and non-squamous histology correlated with better survival.
  • Supports pembrolizumab use in selected populations, even in resource-limited settings.
