Summary

Radiographic angle measurements for pediatric flat feet show moderate reliability in children aged 3-7 years. The AP talus - 1st metatarsal (APT1MT) and calcaneus inclination (CI) angles demonstrated high reliability for assessing this common foot deformity.

Background:

  • Flat feet (pes planus) is a prevalent pediatric foot deformity.
  • Radiographic evaluations are frequently employed for diagnosing flat feet in children.
  • Limited data exists on the reliability of specific radiographic measurements in the 3-7 year age group.

Purpose of the Study:

  • To assess the interobserver and intraobserver reliability of seven radiographic angles used in evaluating pediatric flat feet.
  • To determine the reliability of these measurements specifically in children aged 3-7 years.

Main Methods:

  • Radiographs of 50 children (aged 3-7) with diagnosed flat feet were analyzed.
  • Seven specific radiographic angles were measured by four independent observers (specialists and residents) in two separate sessions.
  • Interclass correlation coefficients (ICCs) were calculated to evaluate interobserver and intraobserver reliability.

Main Results:

  • High reliability was observed for the AP talus - 1st metatarsal (APT1MT) angle and calcaneus inclination (CI) angle.
  • Moderate reliability was found for the remaining five radiographic angles.
  • Intraobserver reliability was generally higher than interobserver reliability.

Conclusions:

  • Radiographic angle measurements for pediatric flat feet exhibit moderate reliability in the 3-7 year age group, with APT1MT and CI angles being exceptions.
  • Observer experience did not significantly influence measurement reliability in this study.
  • Findings highlight the importance of considering measurement variability when interpreting radiographic assessments of flat feet in young children.
