    • High ProGRP (>69.2 pg/mL) showed a time-dependent association with increased mortality risk beyond 10 months (HR=1.85).

    Conclusions:

    • Previously identified risk factors retain prognostic value in older ES-SCLC patients.
    • Age, treatment, smoking history, metastasis, LDH, and ProGRP are crucial for prognosis in this group.
    • The prognostic impact of ProGRP is time-dependent, becoming significant after the initial 10 months of follow-up.
