Targeting HO-1 as a therapeutic strategy against chronic pain

Abstract:
Chronic pain, a leading global health issue, severely compromises patients' quality of life. Its complex pathogenesis involves peripheral and central sensitization processes and is closely associated with a series of inflammatory and oxidative stress (OS) responses. Heme oxygenase-1 (HO-1), a key antioxidant enzyme that extensively regulates cellular OS and inflammatory responses, holds promising therapeutic potential for chronic pain due to its significant anti-inflammatory and neuroprotective effects. This review systematically explores the pathogenesis of chronic pain, detailing its pathological progression, describing the physicochemical properties of HO-1 with emphasis on its anti-inflammatory and antioxidant characteristics, and summarizing multiple regulatory pathways. Challenges in the field, including potential toxic side effects of HO-1 revealed in some studies, are also highlighted. Finally, potential therapeutic drugs targeting HO-1 for chronic pain treatment from 2020 to 2026 are compiled, aiming to provide new perspectives and references for future therapeutic strategies and clinical translation.
