Atypical isolated inner retinal neovascularisation associated with idiopathic epiretinal membrane

Idiopathic vascular epiretinal membrane (ERM) is a macular disorder which is an extremely rare entity. A hypertensive male in his early 50s presented with complaints of distortion and diminished vision in the left eye. Fundus examination showed a vascularised ERM in the right eye, confirmed by a non-invasive imaging modality, optical coherence tomography (OCT) and optical coherence tomography angiography (OCTA). Despite the presence of vascularised ERM, the patient maintained a stable vision of 6/9 in his right eye. Left eye revealed subretinal fluid and double layer sign on OCT. OCTA confirmed the diagnosis of polypoidal choroidal vasculopathy for which the patient was treated with intravitreal anti-vascular endothelial growth factor injection. Patient was asymptomatic in the right eye. This is a novel case of vascularised ERM with less significant effect on visual acuity in the absence of any identifiable cause.
