    Pathological examination of internal capsule infarcts revealed obstructive vascular lesions in nine of ten cases. Atherosclerosis was the primary cause, leading to pure motor hemiplegia or hemiparesis.

    Background:

    • Internal capsule infarcts are a significant cause of neurological deficits.
    • Understanding the underlying vascular pathology is crucial for diagnosis and treatment.

    Observation:

    • Pathological examination of 11 internal capsule infarcts in 10 patients.
    • Detailed analysis of basal ganglia serial sections and gross dissections.
    • Tracing of implicated penetrating arteries to identify vascular lesions.

    Findings:

    • Obstructive vascular lesions were identified in 9 out of 10 cases.
    • Atherosclerosis, including atheromatous plaques with thrombus or stenosis, was the predominant finding.
    • Lipohyalinosis and embolic events were also observed as causes of obstruction.
    • The clinical presentation consistently involved pure motor hemiplegia or hemiparesis without sensory or cognitive deficits.

    Implications:

    • Atherosclerosis of penetrating arteries is a primary cause of internal capsule infarcts.
    • The findings highlight the importance of vascular risk factor management in preventing such strokes.
    • Pure motor deficits strongly suggest lacunar infarcts in the internal capsule.
