Purpose of the Study:

  • To assess racial and ethnic disparities in SDM among prostate cancer patients in the U.S.
  • To identify specific demographic groups with suboptimal SDM experiences.
  • To inform strategies for improving patient-centered care in prostate cancer.

Main Methods:

  • Cross-sectional analysis of 2014-2021 Medical Expenditure Panel Survey (MEPS) data.
  • Inclusion of male patients aged 18+ diagnosed with prostate cancer.
  • Operationalization of SDM using Consumer Assessment of Healthcare Providers and Systems (CAHPS) scores (poor, average, optimal) and multinomial regression analysis.

Main Results:

  • Identified 1.39 million prostate cancer patients; predominant races were White (64.4%), African American (20.65%), and Hispanic (10.60%).
  • Overall, 8.62% reported poor SDM, 49.82% average, and 41.56% optimal.
  • Hispanic patients had significantly higher odds of poor SDM compared to White patients (aOR: 3.23). No significant differences were found for African American patients.

Conclusions:

  • Approximately 1 in 10 prostate cancer patients experience poor SDM.
  • Hispanic patients face a disproportionately higher likelihood of poor SDM.
  • Enhanced cultural competency and communication strategies are vital to improve SDM for Hispanic prostate cancer patients.
