Primary arthrodesis via retrograde tibiotalocalcaneal nailing offers a load-stable fixation for geriatric ankle fractures, enabling immediate weight-bearing and potentially reducing complications. Further prospective studies are needed to confirm its benefits over anatomical reconstruction.
