    Splenectomy for unexplained splenomegaly can aid diagnosis and treatment in some cases. However, spleen removal may not benefit patients with only mild spleen enlargement.

    Background:

    • Splenectomy is a surgical procedure involving the removal of the spleen.
    • Unexplained splenomegaly (enlarged spleen) can present diagnostic and therapeutic challenges.

    Observation:

    • A 9-year study (1968-1976) analyzed 116 splenectomies, with 13 performed for unexplained splenomegaly.
    • Diagnoses were established in 6 patients post-operatively, including sarcoidosis, splenic cysts, Gaucher's disease, and hemangiosarcoma.
    • Histological examination of excised spleens in 7 patients revealed no specific diagnostic features.

    Findings:

    • Two patients with very large spleens experienced significant benefit from splenectomy.
    • One patient developed lymphosarcoma 21 months post-splenectomy.
    • Four patients with mild to moderate splenomegaly showed no clear diagnostic or therapeutic advantages.

    Implications:

    • Splenectomy is a consideration for unexplained moderate to gross splenomegaly.
    • The procedure may offer limited benefits for patients with mild splenomegaly.
