Robotic intracorporeal ileal neobladder: Our modifications of the Karolinska Institute technique

Abstract:
Robotic intracorporeal neobladder reconstruction aims to balance functional outcomes with reduced morbidity and improved cosmesis, but the inherent complexity of the procedure continues to challenge its routine adoption. We describe our modifications to the Karolinska intracorporeal ileal neobladder technique following radical cystectomy. A 52-year-old male with muscle-invasive high-grade urothelial carcinoma underwent robotic radical cystectomy with intracorporeal neobladder diversion. Operative time was 8 h with minimal blood loss and no intraoperative complications. Recovery was uneventful, with satisfactory early daytime continence achieved by 1 month. Our technique demonstrates promising feasibility, reduced complexity, and acceptable early functional outcomes, supporting its applicability for wider surgical adoption.
