Purpose of the Study:

  • To determine the outcomes of varenicline sampling in a randomized clinical trial (RCT) for adults who smoke.
  • To compare varenicline sampling with nicotine replacement therapy (NRT) and quitline referral.

Main Methods:

  • A decentralized RCT involving 651 non-treatment seeking AWS in South Carolina.
  • Participants were randomized to receive 4-week samples of varenicline, NRT (active control), or quitline referral (inactive control).
  • Primary outcome was self-reported 7-day point prevalence abstinence (PPA) at 6-month follow-up.

Main Results:

  • Varenicline sampling led to higher self-reported PPA (17% vs 10%) and floating abstinence (34% vs 20%) compared to the no-sampling control.
  • Varenicline sampling was superior to NRT for 6-month self-reported abstinence (17% vs 8%) and reduction in cigarettes per day (CPD) at week 8.
  • No significant difference in carbon monoxide (CO)-verified abstinence at 6 months between varenicline and no-sampling control.

Conclusions:

  • Varenicline sampling is an efficacious strategy for smoking cessation in adults who smoke.
  • Medication sampling offers a pragmatic approach to engage AWS in quitting, with potentially superior outcomes to NRT.
  • Further evaluation of medication sampling in applied settings is warranted.
