Background:

Paclitaxel premedications prevent infusion hypersensitivity reactions (iHSRs) but cause side effects like sleep disturbance and weight gain.
Previous phase II data suggested discontinuing premedications might be safe after iHSRs, but lacked prospective randomized trials.

Purpose of the Study:

To evaluate the safety and feasibility of discontinuing paclitaxel premedications in patients who have not experienced iHSRs.
To compare the incidence of iHSRs, side effects, and quality of life between patients who continue and discontinue premedications.

Outcomes included subsequent iHSRs, weight changes, drug side effects, and quality of life.

Main Results:

98 patients were randomized; 89 were evaluable. Only 1 patient (2.3%) in the investigational arm had an iHSR, which resolved with treatment.
No iHSRs occurred in the control arm (0.0%).
The investigational arm reported significantly less sleep disruption and appetite increase compared to the control arm.

Conclusions:

Discontinuing paclitaxel premedications after two uneventful doses is safe and associated with fewer steroid-related side effects.
This strategy can reduce unnecessary medication, improve patient experience, and streamline clinical workflows.
Discontinuing premedications appears safe and improves tolerability for patients without prior iHSRs.
