Quantitative native T1 mapping identifies papillary muscle and intracavitary cardiac lipomas: A multiparametric CMR

Abstract:
Cardiac lipomas are rare benign tumors composed of mature adipose tissue. Although typically identifiable using conventional cardiac magnetic resonance (CMR) sequences, diagnostic uncertainty may arise when lesions are small, located in anatomically complex regions, or mimic other intracardiac masses. We report 2 cases of cardiac lipoma with different anatomical presentations: a papillary muscle lipoma in a 47-year-old woman and an intracavitary left ventricular lipoma in a 31-year-old Asian man. In both cases, conventional CMR demonstrated characteristic fat signal features, while native T1 mapping revealed markedly reduced T1 values (250 ms and 405 ms at 1.5 T, respectively), enabling confident tissue characterization. These cases highlight the incremental value of parametric mapping as a quantitative, reproducible tool for diagnosing cardiac lipomas across diverse anatomical locations.
