Summary

Aortic valve repair (AVRep) and valve-sparing root (VSR) procedures demonstrate low early mortality and excellent long-term survival. Older age, female sex, aortic dissection, and isolated valve repair predict higher mortality in these valve-preserving interventions.

Purpose of the Study:

  • To systematically review and meta-analyze current evidence on AVRep and VSR.
  • Identify predictors of mortality and reintervention following these procedures.

Main Methods:

  • Systematic review and meta-analysis of studies with ≥200 adult patients and ≥5 years follow-up.
  • Reconstruction of individual patient data from Kaplan-Meier curves for long-term analysis.
  • Mixed-effects Cox frailty model to identify predictors of mortality and reoperation.

Main Results:

  • 48 studies included 20,914 patients; mean age 52.42 years.
  • Low early outcomes: hospital mortality 1.34%, bleeding 3.77%, neurologic events 1.27%.
  • Excellent long-term survival (20-year: 67%) and freedom from reoperation (20-year: 77.5%). Older age, female sex, aortic dissection, and isolated valve repair linked to higher mortality.

Conclusions:

  • AVRep and VSR offer low early morbidity/mortality and excellent long-term results.
  • Heterogeneity in study definitions necessitates standardized reporting.
  • Further randomized trials are needed to confirm findings.
